WHAT HAPPENED?
On December 29, 2022, CentraState Healthcare System discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, CentraState Healthcare System determined that an unauthorized actor may have accessed and obtained this sensitive information on December 29, 2022. On February 10, 2023, CentraState Healthcare System began contacting individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Health insurance information
- Medical record number
- Patient account number
- Medical and treatment information (e.g., date(s) of service, physician names and departments, treatment plans, diagnoses, visit notes, and prescription information)
