WHAT HAPPENED?
On May 16, 2022, Avem Health Partners’ information technology vendor, 365 Data Centers, discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Avem Health Partners determined that an unauthorized actor may have accessed this sensitive information before May 14, 2022. On December 14, 2022, Avem Health Partners began contacting individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Address
- Driver’s license number
- Health insurance information
- Medical information (e.g., diagnosis or treatment information)
