Established in 1972, Gateway Rehab has been offering patients individualized and care to sustain recovery. Apart of its desire to treat the whole person, Gateway Rehab provides medications, a variety of inpatient and outpatient therapies, and telehealth sessions. Headquartered in Moon Township, Pennsylvania, Gateway Rehab serves the greater Pittsburgh, western Pennsylvania, and Ohio areas by operating a number of clinics and campuses. As of today, Gateway Rehab has treated over 100,000 patients and currently employs more than 500 individuals.
WHAT HAPPENED?
On June 13, 2022, Gateway Rehab discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Gateway Rehab determined that an unauthorized actor may have accessed this sensitive information. On November 18, 2022, Gateway Rehab began notifying individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license number
- Financial information (e.g., account number)
- Health insurance information
- Medical information
