OrthoNebraska is a physician-owned medical practice group operating orthopedic clinics, orthopedic urgent care clinics, and an orthopedic hospital in the Nebraska area. OrthoNebraska’s orthopedic hospital specializes in the diagnostic, surgical, and rehabilitative needs of persons with musculoskeletal conditions. Based in Omaha, Nebraska, OrthoNebraska has 7 clinic locations in Oakview, Arksarben, Council Bluffs, Elkhorn, Fairfax (Missouri), Norfolk, and Sarpy County. OrthoNebraska was created by the mergers of OrthoWest, NebOrtho, and Nebraska Orthopaedic Hospital in 2014 and 2017. OrthoWest, PC, and Nebraska Orthopaedic Hospital, LLC each operate under the name OrthoNebraska.
WHAT HAPPENED?
On or about December 7, 2021, OrthoNebraska experienced a data breach in which the sensitive personal identifiable information and protected health information of OrthoNebraska patients may have been accessed. Recently, OrthoNebraska began notifying individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Address
- Telephone number
- Date of birth
- Driver’s license number
- State identification card number
- Usernames and passwords
- Clinical information
- Health insurance information
- Claim information
