WHAT HAPPENED?
Recently, Bellin Health discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Bellin Health determined that an unauthorized actor may have accessed and acquired this sensitive personal information on October 27, 2023. Bellin Health’s investigation also determined that the data breach involved information related to patients who purchased home care equipment between 2006 and 2013. On December 19, 2023, Bellin Health began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Phone number
- Date of birth
- Health information
