Turke & Strauss LLP, a leading data breach law firm, is investigating Neuromusculoskeletal Center of the Cascades, PC and Cascade Surgicenter, LLC (collectively, “The Center”) regarding its recent data breach. The Center data breach involved sensitive personal identifiable information and protected health information belonging to an undetermined number of individuals.
ABOUT THE CENTER:
The Center is an orthopedic and neurosurgical care and research facility in Oregon. Founded in 1999, The Center’s team of orthopedic, neurosurgical, occupational medicine, and physiatry doctors offer a wide range of services, including concussion treatment, diagnostic imaging, neurosurgery, orthopedic surgery, occupational medicine, pediatric services, joint replacement, sports medicine, and more. Based in Bend, Oregon, The Center has two other locations serving Central Oregon.
WHAT HAPPENED?
On October 3, 2023, The Center discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, The Center determined that an unauthorized actor may have accessed this sensitive information through employee email accounts between October 2, 2023, and October 3, 2023. On December 1, 2023, The Center began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Contact information (e.g., phone number, email address)
- Driver’s license or State ID number
- Financial information (e.g., account number, routing number, institution name, credit/debit card information)
- Medical information (e.g., treatment/diagnosis information, prescription information, provider name, medical record number)
- Health insurance information (e.g., Medicare/Medicaid ID number, treatment cost)
- Digital signature
