ABOUT PEAKMED:
PeakMed is a membership based medical practice offering primary care, urgent care, and pediatric care. As a membership based medical facility, PeakMed provides healthcare services to its members for a flat monthly fee, giving them unlimited access to PeakMed services. As a direct primary care provider, PeakMed serves individuals as well as employers to obtain affordable healthcare without copays or deductibles. Headquartered in Colorado Springs, Colorado, PeakMed has an additional location in Denver, Colorado and employs approximately 50 individuals.
WHAT HAPPENED?
On August 30, 2023, PeakMed discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, PeakMed determined that an unauthorized actor may have accessed and acquired this sensitive information between July 24 and August 30, 2023. On October 27, 2023, PeakMed began contacting individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license number
- Address
- Financial account number
- Medical record number
- Health records
- Health insurance information
