WHAT HAPPENED?

On March 29, 2023, Vascular Center of Intervention discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Vascular Center of Intervention determined that an unauthorized actor may have accessed and obtained this sensitive information between February 25 and March 29, 2023. On May 24, 2023, Vascular Center of Intervention began contacting individuals whose information may have been impacted. The type of information exposed includes:

  • Name
  • Contact information
  • Social Security number
  • Driver’s license information
  • Date of birth
  • Medical information (e.g., medical history, mental or physical condition, medical treatment or diagnosis)
  • Health insurance information
