CINQCARE is a provider-led healthcare organization focused on providing care to underserved communities. Comprised of physicians, nurse practitioners, and physician assistants, CINQCARE offers a person-centered system of care to coordinate and provide access to health and care, including mental health and community-based services to improve the health and well-being of members.2 These services include CINQCARE’s ACO Reach program, Health Home program, Care at Home services, transportation, food and housing, one-call nursing solutions, advanced clinical monitoring, and additional hospital support. Headquartered in Washington, D.C., CINQCARE has two additional locations in New York and employs over 200 individuals.
WHAT HAPPENED?
Recently, CINQCARE discovered that it had experienced a data beach in which sensitive personal information in its systems may have been accessed. On January 4, 2024, CINQCARE began notifying individuals whose information may have been impacted. The exact type of personal information potentially exposed has not been made publicly available by CINQCARE. However, according to state reporting guidelines, “personal information” can include the following types of information:
- Name
- Social Security number
- Driver’s license or state-issued identification number
- Financial information (e.g., financial account number, credit or debit card number)
