WHAT HAPPENED?
Recently, Community Healthcare Network discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Community Healthcare Network determined that an unauthorized party may have accessed and acquired this sensitive information during a major network disruption on October 1, 2023. When its investigation is complete, Community Healthcare Network will directly notify individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Date of birth
- Addresses and phone numbers
- Health insurance information, including ID number
- Patient ID number
- Provider name and service date
