Headquartered in Marshfield, Wisconsin, Forward Bank has 14 additional locations throughout Wisconsin and operates more than 32,000 ATMs.
WHAT HAPPENED?
On September 10, 2023, Forward Bank discovered that it had experienced a data breach in which sensitive personal identifiable information in its systems may have been accessed and acquired. Through its investigation, Forward Bank determined that an unauthorized actor may have accessed and acquired this sensitive information on or around September 6, 2023. On November 17, 2023, Forward Bank began notifying individuals whose information may have been impacted by the breach. The exact type of personal information potentially exposed has not been made publicly available by Forward Bank. However, according to state reporting guidelines, “personal information” can include the following types of information:
- Name
- Social Security number
- Driver’s license number
- Financial account number
- Credit or debit card number
- Unique biometric data
