Accordingly, Clean Uniforms produces a wide range of uniform types, including a variety of shirts, pants, coverings, coats, shorts, outwear, kitchen wear, and other specialty garments. In addition to uniforms, Clean Uniforms also offers customized towels, cleaning cloths, mats, and mops as well as facility services such as lockers and storage, dispensers, odor control, and more. Headquartered in New Bedford, Massachusetts, Clean Uniforms is a third-generation family-owned business that employs over 10 individuals.
WHAT HAPPENED?
On or around January 22, 2023, Clean Uniforms discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Clean Uniforms determined that an unauthorized actor may have accessed this sensitive information. Recently, Clean Uniforms began contacting individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Address
- Date of birth
- Driver’s license number
- Bank account and routing number
- 410k participant ID number
- Credit card information
- Medical information
- Health insurance policy ID number
