WHAT HAPPENED?
In early December 2022, Synergy Healthcare Services discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Synergy Healthcare Services determined that an unauthorized actor may have accessed this sensitive personal identifiable information and protected health information. On July 27, 2023, Synergy Healthcare Services began contacting individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Date of birth
- Driver’s license number
- Contact information
- Medical information (e.g., medical treatment, medical history)
- Health insurance information
- Financial information (e.g., bank account number)
