Arizona Priority Care Data Breach Investigation

Turke & Strauss LLP, a leading data breach law firm, is investigating Arizona Health Advantage, Inc. d/b/a Arizona Priority Care and AZPC Clinics, LLC (collectively, “APC”) regarding its recent data breach. The APC data breach involved sensitive personal identifiable information and protected health information belonging to over 10,900 individuals.

APC is a fully integrated medical group that partners with health plans to offer patients a comprehensive approach to their healthcare needs. Accordingly, APC is made up of various primary care physicians and specialists to ensure coordinated patient care under health plans APC contracts. While APC was founded in 2012, its affiliate, Heritage Provider Network (HPN), was established in 1979 by founder Dr. Richard Merkin. Headquartered in Chandler, Arizona, APC prides itself on patient care, handling referrals, and handling claims and case management efficiently.

WHAT HAPPENED?

On December 5, 2022, APC discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, APC determined that an unauthorized actor may have accessed this sensitive information between December 1 and December 2, 2022. Recently, APC released a breach notice detailing its plans to contact individuals whose information may have been impacted. The type of information exposed includes:

· Name

· Address

· Date of birth

· Health insurance information (e.g., health plan member number)

· Medical information (e.g., treatment dates and information, services authorization numbers)

  6. Contact a credit bureau(s) to request a temporary fraud alert.
