WHAT HAPPENED?
On or around November 2, 2022, Precision Imaging Centers discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and acquired. Through its investigation, Precision Imaging Centers determined that an unauthorized actor may have accessed and obtained this sensitive information on or around November 2, 2022. On June 22, 2023, Precision Imaging Centers began contacting individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Driver’s license or government issued identification number
- Address
- Date of birth
- Health insurance information
- Medical conditions and diagnoses
- Health- or medical-related information
