Located throughout northeastern and central Pennsylvania, the Commonwealth Health Physician Network represents a variety of different specialties in addition to cardiology including pediatrics, neurology, general surgery, and family medicine. With twelve providers, Great Valley Cardiology is located in Scranton, Pennsylvania and offers telehealth appointments in addition to in-person visits. As a heart care clinic, Great Valley Cardiology provides care and treatment for conditions including coronary artery disease, congestive heart failure, and peripheral vascular disease. Accordingly, Great Valley Cardiology offers a range of procedures such as open-heart surgery, defibrillator implantation, and thoracic aneurysm repair.
WHAT HAPPENED?
On April 13, 2023, Great Valley Cardiology discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Great Valley Cardiology determined that an unauthorized actor may have accessed this sensitive information between February 2, 2023 and April 14, 2023. On June 12, 2023, Great Valley Cardiology began contacting individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Date of birth
- Address
- Driver’s license and passport number
- Credit card or debit card information
- Health insurance information
- Medical information (e.g., treatment or diagnosis information)
