WHAT HAPPENED?
On April 5, 2023, South Jersey Behavioral Health Resources discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, South Jersey Behavioral Health Resources determined that an unauthorized actor may have accessed this sensitive information beginning on April 3, 2023. Recently, South Jersey Behavioral Health Resources posted a notice of the data breach on its website describing the breach and the type of information potentially impacted. The type of information exposed includes:
- Name
- Social Security number
- Driver’s license number
- Date of birth
- Medical record number
- Treating physician
- Health insurance information
- Subscriber number
- Medical history information
- Diagnosis and treatment information
