WHAT HAPPENED?
On April 21, 2022, Little Colorado Medical Center discovered that it had experienced a data breach in which the sensitive personal identifiable information in its systems may have been accessed. Through its investigation, Little Colorado Medical Center determined that an unauthorized actor may have accessed this sensitive information between March 7 and April 21, 2022. On May 25, 2023, Little Colorado Medical Center began contacting individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Address
- Social Security number
- Driver’s license number
