WHAT HAPPENED?
On January 1, 2023, Flatirons Solutions discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Flatirons Solutions determined that an unauthorized actor may have accessed this sensitive information between December 15, 2022, and January 1, 2023. On April 10, 2023, Flatirons Solutions began notifying individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Date of birth
- Financial account number
- Health insurance information
