WHAT HAPPENED?
On October 7, 2022, Integrated Supports for Living discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Integrated Supports for Living determined that an unauthorized actor may have accessed this sensitive information between July 26 and October 7, 2022. On March 21, 2023, Integrated Supports for Living began contacting individuals whose information may have been impacted. The type of information exposed includes:
- Name
- Social Security number
- Date of birth
- Birth certificate
- Driver’s license number
- Passport number
- Health insurance information
- Financial information
- Medical information (i.e., medical history, clinical information, mental or physical condition or treatment)
- Username and password
