WHAT HAPPENED?
On October 21, 2022, Lake Charles Memorial Health discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed and obtained. Through its investigation, Lake Charles Memorial Health System determined that an unknown actor may have accessed sensitive information between October 20 and October 21, 2022. On December 23, 2022, Lake Charles Memorial Health System began mailing letters to patients whose information may have been involved in this incident. The types of information exposed includes:

- Social Security number
- Address
- Date of birth
- Medical record or patient identification numbers
- Health insurance information
- Payment information
- Clinical information
