Established in 1976, Bay Oral serves patients in need of dental implants, bone grafting, wisdom teeth removal, dental extractions, corrective jaw surgery, and other oral surgery services.2 Comprised of multiple board-certified oral surgeons, Bay Oral provides a full scope of oral and maxillofacial surgery that has been trusted by dentists and patients for over 40 years.2 Headquartered in Green Bay, Wisconsin, Bay Oral has two additional locations in Wisconsin.

WHAT HAPPENED?

On February 27, 2024, Bay Oral discovered that it had experienced a data breach in which sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Bay Oral determined that an unauthorized actor may have accessed this information through an employee’s email account on January 18, 2024. On April 25, 2024, Bay Oral began notifying individuals whose information may have been impacted. The type of information potentially exposed includes:

  • Name
  • Social Security number
  • Adress
  • Date of birth
  • Contact information
  • Financial information (e.g., bank account information, credit card number)
  • Medical information (e.g., patient visit summary notes, x-rays, patient health history forms, medical history questionnaires) 
  • Insurance card number
